Print the completed FL Fingerprint Submission Form and sign in required signature areas.
Go to your local police/sheriff agency that conducts public fingerprinting and have two separate sets of fingerprints (not a copy of the original for the second set) rolled on standard FD258 cards.
✓ Signed FL Fingerprint Submission form.
✓ Two fingerprint sets.
✓ One copy/form of government issued ID.
✓ Payment: money order, cashiers check, or online payment. (If paying online, you must download and print your receipt after checkout and include with your application).
